Orphanet
Etiology The pathogenesis still remains unknown, although an association has been reported with 22q11.2 deletion syndrome in children with AOLPA, usually associated with Right aortic arch, Tetralogy of Fallot and Absent pulmonary valve. ... Genetic counseling Abnormal origin of right or left pulmonary artery from aorta is usually non-syndromic and sporadic. However, association with the 22q11.2 deletion syndrome is described in patients with AOLPA.

Delirium Tremens
Wikipedia
Specialty Psychiatry , critical care medicine Symptoms Confusion , hallucination , shaking , shivering, irregular heart rate , sweating [1] [2] Complications Very high body temperature , seizures [2] Usual onset Rapid [2] Duration 2–3 days [2] Causes Withdrawal from alcohol [2] Differential diagnosis Benzodiazepine withdrawal syndrome , barbiturate withdrawal [3] Treatment Intensive care unit , benzodiazepines , thiamine [2] Prognosis Risk of death ~2% (treatment), 25% (no treatment) [4] Frequency ~4% of those withdrawing from alcohol [2] Delirium tremens ( DTs ) is a rapid onset of confusion usually caused by withdrawal from alcohol . [2] When it occurs, it is often three days into the withdrawal symptoms and lasts for two to three days. [2] Physical effects may include shaking , shivering, irregular heart rate , and sweating. [1] People may also hallucinate . [2] Occasionally, a very high body temperature or seizures may result in death. [2] Alcohol is one of the most dangerous drugs from which to withdraw . [5] Delirium tremens typically only occurs in people with a high intake of alcohol for more than a month. [6] A similar syndrome may occur with benzodiazepine and barbiturate withdrawal . [3] Withdrawal from stimulants such as cocaine do not have major medical complications. [7] In a person with delirium tremens it is important to rule out other associated problems such as electrolyte abnormalities , pancreatitis , and alcoholic hepatitis . [2] Prevention is by treating withdrawal symptoms. [2] If delirium tremens occurs, aggressive treatment improves outcomes. [2] Treatment in a quiet intensive care unit with sufficient light is often recommended. [2] Benzodiazepines are the medication of choice with diazepam , lorazepam , chlordiazepoxide , and oxazepam all commonly used. [6] They should be given until a person is lightly sleeping. [2] The antipsychotic haloperidol may also be used. [2] The vitamin thiamine is recommended. [2] Mortality without treatment is between 15% and 40%. [4] Currently death occurs in about 1% to 4% of cases. [2] About half of people with alcoholism will develop withdrawal symptoms upon reducing their use. [2] Of these, 3% to 5% develop DTs or have seizures. [2] The name delirium tremens was first used in 1813; however, the symptoms were well described since the 1700s. [6] The word "delirium" is Latin for "going off the furrow," a plowing metaphor. [4] It is also called shaking frenzy and Saunders-Sutton syndrome. [4] Nicknames include the shakes, barrel-fever, blue horrors, bottleache, bats, drunken horrors, elephants, gallon distemper, quart mania, heebie jeebies, pink spiders and riding the ghost train. [8] Contents 1 Signs and symptoms 2 Causes 3 Pathophysiology 4 Diagnosis 5 Treatment 6 Society and culture 7 See also 8 References 9 External links Signs and symptoms [ edit ] The main symptoms of delirium tremens are nightmares, agitation, global confusion, disorientation, visual and [9] auditory hallucinations, tactile hallucinations , fever, high blood pressure , heavy sweating , and other signs of autonomic hyperactivity ( fast heart rate and high blood pressure). ... Postnatal diagnosis [ edit ] If patients aren’t diagnosed with dilation of their urinary tract via ultrasound in utero, they can present after birth with vague symptoms such as abdominal pain, blood in their urine or a urinary tract infection. [3] Associated syndromes [ edit ] If patients have other congenital anomalies, their bladder outlet obstruction may be recognized during evaluation for their related syndromes. For example, VACTERL association is a constellation of congenital anomalies including vertebral, anal, cardiac, tracheoesophageal, renal and limb defects. Prune belly syndrome (or Eagle-Barrett syndrome) is another group of congenital disorders that involves the kidneys and includes absent abdominal wall musculature, severe urinary tract abnormalities and bilateral undescended testicles. [4] Dietl crisis [ edit ] Patients with an undiagnosed ureteropelvic junction obstruction may experience abdominal or flank pain after increased fluid intake, when their bladder is full or when they exercise. [1] Terminology [ edit ] Bladder neck obstruction is a condition where the bladder neck does not open enough during voiding . [5] Congenital Anomalies of the Kidney and Urinary Tract (CAKUT) [ edit ] Bladder outlet obstruction is included in the spectrum of congenital anomalies of the kidney and urinary tract (CAKUT).PTGS2, ADRA1A, NGF, COL3A1, SOD2, NOS3, MMP7, OPTN, HIF1A, ELN, HTR2A, COL1A1, NLRP3, GABPA, IL1B, NFE2L2, PPP1R14A, UBC, POU5F1P3, MIR29C, TGFB1, TIMP1, TNF, TYRP1, MIR29B2, MIR29B1, MIR132, NRP2, VEGFA, PARPBP, NPEPPS, LRIG2, BCL2L10, SLC22A1, MYL12B, MYL12A, WWTR1, PSAT1, SLC22A3, POU2F1, SLC5A2, NRG1, AGT, AKT1, KLK3, BDNF, CASP3, CNR1, CNR2, CYP19A1, DCN, ESR1, GATA6, IL1A, RPS27A, KCNK2, LOX, SMAD3, MECP2, MET, MMP1, PLAG1, ADRB3, POU5F1, PROS1, PTEN, POU5F1P4
