Selective serotonin reuptake inhibitors , commonly referred to as SSRIs, are considered first line pharmacotherapy for depression in late life as they are more tolerable and safer than other antidepressants. [20] Serotonin-norepinephrine reuptake inhibitors (SNRIs) are considered second-line but also can be useful for patients suffering from chronic pain. [21] [22] Atypical antidepressants such as bupropion and mirtazapine have not been studied extensively in older adults but appear to offer some benefit. [23] [24] Monoamine oxidase inhibitors (MAOIs) similarly have been shown to offer some benefit, but have not been studied extensively [25] MAOIs must be used with caution to prevent side effects such as serotonin syndrome and adrenergic crisis . [26] Tricyclic antidepressants are no longer the first line therapy for depression, but can still benefit patients who do not respond to initial therapies. [22] TCAs have also demonstrated a unique ability to prevent re-occurrence of depression following electroconvulsive therapy. Steroids do not prevent the diagnosis from later being confirmed by biopsy, although certain changes in the histology may be observed towards the end of the first week of treatment and are more difficult to identify after a couple of months. [24] The dose of corticosteroids is generally slowly tapered over 12–18 months. [25] Oral steroids are at least as effective as intravenous steroids, [26] except in the treatment of acute visual loss where intravenous steroids appear to offer significant benefit over oral steroids. [27] Short-term side effects of prednisone are uncommon but can include mood changes, avascular necrosis , and an increased risk of infection. [28] Some of the side effects associated with long-term use include weight gain, diabetes mellitus , osteoporosis , avascular necrosis, glaucoma , cataracts , cardiovascular disease, and an increased risk of infection. [29] [30] It is unclear whether adding a small amount of aspirin is beneficial or not as it has not been studied. [31] Injections of tocilizumab may also be used. [32] Tocilizumab is a humanized antibody that targets the interleukin-6 receptor, which is a key cytokine involved in the progression of GCA. [33] Tocilizumab has been found to be effective at minimizing both recurrence, and flares of GCA when used both on its own and with corticosteroids. [33] Long term use of tocilizumab requires further investigation. [33] [34] Tocilizumab may increase the risk of gastrointestinal perforation and infections , however it does not appear that there are more risks than using corticosteroids. [4] When there are more than four recurrent episodes of candidal vulvovaginitis per year, a longer initial treatment course is recommended, such as orally administered fluconazole followed by a second and third dose 3 and 6 days later, respectively. [27] Other treatments after more than four episodes per year, may include ten days of either oral or topical treatment followed by fluconazole orally once per week for 6 months. [25] About 10-15% of recurrent candidal vulvovaginitis cases are due to non- Candida albicans species. [28] Non- albicans species tend to have higher levels of resistance to fluconazole. [29] Therefore, recurrence or persistence of symptoms while on treatment indicates speciation and antifungal resistance tests to tailor antifungal treatment. [27] Alternative medicine [ edit ] Up to 40% of women seek alternatives to treat vaginal yeast infection. [30] Example products are herbal preparations, probiotics and vaginal acidifying agents. [30] Other alternative treatment approaches include switching contraceptive, treatment of the sexual partner and gentian violet .
